The community health workers are a team of professionals dedicated to helping members:
- Get connected to community resources.
- Access the health care they need.
- Overcome transportation, housing, food instability and other social barriers.
Community health workers will:
- Meet with you face-to-face to help address your needs.
- Guide you through the health care system through educational resources and community coordination.
- Help you understand your health conditions and get you connected to our Care Management program.
Meeting You Where You Are
Community health workers provide personalized support and quick help when and where you need it. Community health workers offer face-to-face meetings where you live, work or receive care.
The community health team can help you:
- Navigate your Healthy Blue benefits.
- Get social support and informal counseling.
- Fill out your personal health assessment (PHA).
- Make a plan to reach your health goals.
- Locate resources near you.
- Learn where and when to go for urgent care.
- Find and set up a primary care provider (PCP).
- Get rides for doctor visits and make appointments.
- Get follow-up care from a provider.
- Understand your discharge plan from a provider.
- Learn about your condition and ways to treat it.
- Find materials so you can learn more about your health and wellness.
