Community Health Workers: Creating Caring Connections

Healthy Blue’s community health workers (CHWs) are here to help our members overcome any health care challenges that might get in the way. Our CHW team’s main goal is to help people stay healthy. They work hard to make sure our Healthy Blue members get the benefits they need to live a healthy life.

Meeting you where you are is how our CHWs give personalized support and quick help when you need it. They can offer face-to-face meetings where you live, work or receive care.

The community health team will help members:

• Learn about your Healthy Blue benefits.
• Obtener apoyo social y asesoría informal.
• Fill out your personal health assessment.
• Get connected to local community resources that can help cover your needs.
• Access the health care you need such as choosing a primary care provider (PCP).
• Saber a dónde y cuándo acudir para recibir atención de urgencia.
• Make medical appointments and schedule rides to PCP visits.
• Get follow-up care from a PCP.
• Obtener información sobre tu afección y las formas de tratarla.
• Offer information about your health and wellness.
• Make a plan to help you reach your health goals.
